26.22.345 Sex offender acknowledgement form.
This section is included in your selections.
(1) The sex offender shall read, or have read to them, and sign a form stating that the duty to register has been explained to them by the Cowlitz Department of Public Safety and that the sex offender understands the registration requirement.
(a) The form shall be signed and dated by the Cowlitz Department of Public Safety personnel registering the sex offender.
(2) The Cowlitz Department of Public Safety shall immediately upload the acknowledgement form into the Cowlitz Department of Public Safety sex offender registry.
